If you are interested in using our facilities for events, sports practices, meetings, etc. please fill out the FACILITY USE form and submit it and your Certificate of Liability to Laura Smith at lsmith@frewsburgcsd.org or mail to PO Box 690, Frewsburg, NY 14738.

After you submit your form, you will be notified as to the availability of your request and then it will be forwarded to the Building Principal for approval. All outside organizations must supply a Certificate of Liability Insurance with their Facility Use Form.
